Mission College is offering the Influenza immunization (flu shot) to any community member who pre-registers. We will provide the vaccine as long as we have supplies and the weather is tolerable. This is not a COVID-19 vaccine.

Forms and Procedure
The forms below allow you to review the risk and benefits of vaccination, you may complete the forms on your computer or smart phone. Return the forms via email to alfred.forrest@missioncollege.edu. Please complete the first patient line on each consent form and provide your name and your birth date or age range as noted. On the WVMCCD consent form, please provide your name and student ID number.

In your email, advise us of your preferred appointment time, we will do our best to accomodate your preference and email you a confirmation time and registration number. Remember to provide us with an accurate email address and phone number. Please have your registration number handy when you arrive.
